Denture Allergies: Residual Monomer, Nickel Sensitivity and What to Ask Your Lab
A denture that causes soreness soon after fitting is often put down to a fit issue — and sometimes that’s exactly what it is. But not always. A genuine material reaction, to either the acrylic or the metal framework, can look similar and is worth knowing how to recognise.
Residual monomer in acrylic
Most denture bases are made from PMMA (polymethyl methacrylate), polymerised from a liquid monomer during processing. When polymerisation is complete, very little monomer remains. But if it’s incomplete, a small amount of unreacted monomer can be left in the finished material and gradually leach out — and this is thought to contribute to some local reactions, including a burning sensation, oral irritation or ulceration in susceptible patients.
This is one reason processing quality matters, not just the raw material — a properly cured acrylic base should carry minimal residual monomer.
Nickel and cobalt-chrome frameworks
Partial dentures commonly use a cobalt-chromium alloy framework for strength. Alongside cobalt and chromium, these alloys can include other elements — nickel among them, depending on the specific alloy. Reactions to metal components are generally linked to corrosion and the release of metal ions in the mouth over time, and tend to appear localised to the tissue in direct contact with the metal — a useful clue when trying to distinguish it from a general fit problem.
Telling a reaction apart from a fit issue
The two can look similar, which is exactly why it’s worth knowing the difference:
- A fit problem usually causes soreness or ulceration localised to a pressure point — where the denture is rubbing or overextended.
- A material reaction more often presents as diffuse burning, redness or swelling across the area the denture contacts, sometimes without an obvious mechanical cause.
Neither rule is absolute, and a proper clinical assessment is what actually distinguishes them — but recognising the pattern is often the first step toward asking the right question.
What’s worth asking your lab
If a material reaction is suspected — or a patient has a known or suspected sensitivity — it’s worth asking your lab directly:
- What base material is being used, and how it’s processed
- What metal alloy, if the case includes a metal framework, and its composition
- Whether material options exist for a case with a flagged sensitivity
A lab working to a proper manufacturing standard should be able to answer all three without difficulty. In the UK, denture materials used by a compliant lab should be CE marked and meet UK medical device regulations — a baseline that doesn’t rule out an individual sensitivity, but does mean the materials themselves are manufactured and tested to a recognised standard.
